The Problem

Where Cardiology Revenue Slips

A follow-up visit billed 99213 when the medical decision making (MDM) supports 99214 is revenue lost. So is an in-office EKG the note documents but the claim leaves out.

Commercial and Medicare payors still expect every billed level to hold up. Brellium audits 100% of charts pre-submission.

How Brellium Works

How Brellium Audits 100% of Your Cardiology Charts

Brellium connects to your EMR and audits every chart automatically, no manual uploads or sampling, the same standard at 50 providers or 500.

01

MDM & E/M Coding

Validate every new (99202-99205) and established (99212-99215) office visit level against the documented MDM.

02

EKG Charge Capture

Match every in-office EKG to a 93000 line on the claim, so tests you perform get paid.

03

Independent Interpretation Credit

Confirm tests the cardiologist reads independently count toward MDM, so the visit bills at the level the work supports.

04

Clawback Protection

Backed by Confidence Coverage: If a Brellium approved document triggers a clawback, Brellium will cover it.

Standard Deployment

  • E/M coding validation (99202-99215)
  • EKG (93000) charge capture
  • Independent-interpretation credit
  • Document-based chart review
  • Confidence Coverage clawback guarantee for notes and documentation

Enterprise Customization

  • Echo (93306/93307), stress test (93015-93018), nuclear (78451-78454), and Holter (93224-93248) coding
  • Device remote monitoring (93293-93298)
  • Cath (e.g., 93458), PCI, EP ablation, and device-implant coding
  • G2211
  • Modifier-25
  • Care management (CCM, TCM, RPM, ACP)
Risk Detection

Common Cardiology Coding Risks Brellium Catches

01

Undercoded Office Visits

Flags visits billed below the level the documented MDM supports.

02

Overcoded Visit Levels

Catches billed levels the MDM documentation doesn't support.

03

Unbilled In-Office EKGs

Catches EKGs documented at the visit with no charge on the claim.

04

Uncredited Independent Interpretations

Flags visits where a test the cardiologist read wasn't counted toward the level.

Built for How Cardiology Coding Works

Visit Level and EKG, Where the Revenue Is

Office cardiology is E/M-weighted, with in-office diagnostics stacked on the visit. Brellium audits the level and the EKG together on every chart.

Accuracy, Not Uplift

Brellium levels each visit to what the documented MDM supports, so recovered revenue is backed by the note if a commercial or Medicare payor reviews it.

FAQ

FAQs About Cardiology Compliance

What are the CPT codes for an EKG?

EKG CPT codes split the service into the tracing and the interpretation:

  • 93000: routine 12-lead EKG with interpretation and report (global)
  • 93005: tracing only, without interpretation and report
  • 93010: interpretation and report only

A practice that performs and reads the EKG in-office bills 93000. A cardiologist reading a tracing performed elsewhere, such as a hospital, bills 93010.

Does an EKG interpretation count toward the MDM level?

An EKG interpretation counts toward MDM only when the cardiologist isn't reporting the test separately. Under the AMA E/M guidelines, independent interpretation credit doesn't apply if 93000 or 93010 is billed for the same EKG.

What coding guidelines apply to cardiology office visits?

Cardiology office visits follow the AMA CPT E/M guidelines revised in 2021, with the level set by MDM or total time on the date of the encounter. For MDM, two of three elements must meet or exceed the level billed:

  • Number and complexity of problems addressed
  • Amount and complexity of data reviewed and analyzed
  • Risk of complications from patient management

Diagnostic tests like EKGs and echocardiograms bill on their own CPT codes on top of the visit.

Does Brellium audit echo, stress test, and cath coding?

Echo, stress test, and cath coding are available as Enterprise Customization rather than standard deployment. That includes echo (93306, 93307), stress testing (93015-93018), and cath (e.g., 93458), along with nuclear, Holter, and device monitoring. The standard deployment covers E/M levels, EKG charge capture, and interpretation credit.

Is Brellium cardiology RCM software?

No. Brellium isn't an RCM or billing platform, and it doesn't submit claims or work denials. It audits 100% of cardiology charts pre-submission, so the E/M levels and EKG charges that reach your billing team match the note.

We use Scribe to create our notes. Do we still need Brellium?

Yes. Scribe helps cardiologists write notes faster, but it doesn't check whether the billed E/M level and EKG charges match what the note documents. Brellium audits the notes your Scribe produces.
