Sumato Health is a readmissions prevention company focused on the skilled nursing phase of post-acute care. By combining advanced AI with licensed nurse oversight, Sumato helps skilled nursing facilities (SNFs) detect early signs of clinical decline and coordinate timely interventions to avoid costly rehospitalizations.
Faced with a fast-growing caseload and increasingly complex CMS compliance requirements, Sumato needed a smarter, scalable solution for managing chronic care management compliance and quality assurance. Their existing manual chart review process was inefficient, error-prone, and a constant source of stress for leadership.

Challenge
Before Brellium, Sumato’s documentation audits were entirely manual. At the end of each month, the team — including leadership — would spend hours (or even days) poring over claims and care notes to meet CMS compliance standards.
“We engaged virtual assistants, our own staff, and honestly just brute-forced our way through QA,” says Sam Weyen, CEO. “We were trying to check every field and bullet point required by CMS ourselves — and mistakes still slipped through.”

Implementation
Though implementation took longer than anticipated due to EMR-related hurdles, Sam credits Brellium’s team — especially their Implementation Manager Kobe — for their persistence and adaptability.
“Our EMR partner had no API access and required custom SFTP and zip file protocols. Brellium adapted to all of it. They never let the project drop, and now we’re fully live and auditing all our notes.”
