Behind the Scenes: The Artificial Intelligence & Human Intelligence Powering Brellium’s E/M Coding

Meet the CPC-certified experts behind Brellium's Coding service — and learn why behavioral health practices trust them to review their notes.

By Olin Wakkary, Head of Services @ Brellium

Accurate evaluation and management (E/M) coding is high stakes. Under-coding visits risks leaving compliant revenue on the table, while over-coded sessions invite compliance risks, audits, and recoupments.

When behavioral health practices process thousands of E/M visits each month, reviewing codes manually can become nearly a full time job.

Providers tell us they are “staying afloat, barely” with manual review, while some clinical leaders have even had to reduce their caseloads to manage the demands of code review.

Brellium's Coding Service was built to take the burden off of providers so they can focus on what matters most - patients. Here's a look at exactly how it works — and why you can trust our coding team to get it right.

What Is Brellium’s Coding Service?

Brellium pairs artificial intelligence and human intelligence to ensure that every note is coded correctly and compliantly, every time.

Our coding software first analyzes every clinical note to check the provider’s E/M coding matches medical decision-making criteria. We're confident in this criteria as it's been trained on over 2 million E/M notes and consistently achieves above 95% inter-rater reliability when compared against clients’ own codes — allowing you to be sure that Brellium-approved notes do not need a manual glance from your team.

If a code is flagged as potentially under- or over-coded, Brellium’s certified medical coders step in to review the note.

They validate whether the CPT code was accurately applied or needs adjustment, then work with providers directly to correct codes needing adjustments.

The end result is accurate, defensible documentation, with no added work for your team.

Who Are Brellium's Coders?

Brellium is dedicated to having the best in the industry review your notes.

Every coder on Brellium's team is a Certified Professional Coder (CPC) — the most recognized medical coding credential in the healthcare industry, issued by the AAPC.

To obtain a CPC, candidates must demonstrate mastery across CPT codes, ICD-10-CM diagnosis codes, modifiers, and coding guidelines in a rigorous four-hour proctored exam. They also must have two additional years of verified, on-the-job coding experience, confirmed by employer letters, and complete regular continuing education.

This means Brellium's coders are continuously learning and keeping pace with code and regulatory changes.

Brellium also actively seeks to hire coders who hold the Certified Evaluation and Management Coder (CEMC) credential — a specialty designation specific to E/M coding. We look for this dual-credential profile to ensure that our coders have seen it all — we want people with broad medical coding knowledge and deep expertise in outpatient behavioral health reviewing flags in your notes.

Our Hiring Process: Rigorous by Design

Brellium holds its coders to the same standard of excellence as its internal team. The selection process includes:

  • Initial screens to verify credentials and role fit
  • Structured interviews assessing soft skills and ability to collaborate with clinical and billing teams
  • A core competency assessment: candidates are given a problem set of real-world clinical notes and asked to assign E/M codes. Their results are benchmarked against industry guidelines and Brellium's established accuracy standards before any hiring decision is made.

Only candidates who pass on all three dimensions — credentials, cultural fit, and demonstrated coding accuracy — join the Brellium team.

Ongoing Performance Standards

Brellium conducts ongoing accuracy audits of code adjustments made by our coding team to ensure continued excellence.

On a regular basis, Brellium samples reviewed notes to verify that coding decisions remain consistent with AMA guidelines and Brellium's internal benchmarks.

If you want to see exactly how your notes are being reviewed and coded, Brellium can share that data with you directly — because we believe that transparency is part of the partnership.
